Referral

You do not need to have a referral to be seen in our office. We however, do advise that you have a dental cleaning and checkup within the last year.

If you are a dentist and would like to refer your patient to our office, please print out the form below and either have your patient bring with them to their appointment or you may fax or email it to us.
